Rooted Renewal is a 12-week, twice-weekly, small-group lifestyle medicine program led by a Nurse Practitioner with team support. You'll cook together, move together, learn the six pillars of lifestyle medicine, and build the habits that support long-term health.

It uses daily habits — nutrition, movement, sleep, stress management, social connection, and the avoidance of risky substances — to prevent, improve, and reverse chronic conditions.

No. Rooted Renewal is a structured, supported reset rooted in clinical evidence, not a diet plan. We teach a whole-food, plant-forward, culturally relevant way of eating you can sustain.

Sessions are held in person in Henry County, Georgia. Specific location details are shared with enrolled participants.

Life happens. We'll share materials and recipes for missed sessions and help you stay on track with your cohort.

No. We do not prescribe, adjust, or discontinue medications. That conversation always stays with your prescribing healthcare provider. We're glad to support you in communicating with your provider as your lifestyle changes take effect.

According to the American College of Lifestyle Medicine, lifestyle interventions can prevent, improve, and even reverse type 2 diabetes, hypertension, and other chronic conditions. Many participants in lifestyle medicine programs experience meaningful improvements in blood sugar, blood pressure, cholesterol, energy, sleep, and weight. Individual results vary.

No referral is required. Rooted Renewal is a wellness education and lifestyle coaching program, not medical care, so we encourage you to keep your primary care relationship active throughout.

Most likely yes — many participants live with chronic conditions. We do ask that you let your healthcare provider know you're joining the program and that you bring any provider-specific guidance with you. -

Important Policy Information

We are committed to delivering a high-quality wellness experience.
Due to the nature of the Rooted Renewal™ program, all payments are non-refundable once the program has begun.
If a participant cancels at least 14 days prior to the program start date, a partial refund may be issued, minus a $110 administrative fee.
No refunds will be issued for:
Missed sessions
Failure to attend
Early withdrawal after the program has started
In the event that a session is canceled by Legacy Creek, a make-up session or credit will be provided.

Individual results may vary. No specific outcomes are guaranteed.
5. Payments
All payments must be made in full or according to the selected payment plan prior to participation.
6. Cancellations & Refunds
Refunds and cancellations are governed by our Refund Policy.
7. Program Changes
Legacy Creek reserves the right to modify program content, schedule, or delivery format as needed.
8. Code of Conduct
Participants are expected to maintain respectful and appropriate behavior in all group settings. Participants are expected to maintain confidentiality of information shared within the group experience.
9. Limitation of Liability
Legacy Creek Community Wellness and its team shall not be held liable for any injuries, losses, or damages resulting from participation in the program.
