Program Agreement

This Program Agreement describes the terms of participation in programs offered by Legacy Creek Community Wellness.

Program Notice

Legacy Creek Community Wellness provides lifestyle medicine education, health coaching, group movement, cooking experiences, gardening activities, and community support. Legacy Creek does not provide primary care, diagnose or treat medical conditions, prescribe or manage medications, or replace care from a participant’s physician or other healthcare professional. Participation does not establish a primary-care provider–patient relationship. Participants should continue routine medical care and consult their healthcare professional before changing medications, diet, physical activity, or treatment plans. Legacy Creek Community Wellness is an independently operated organization and is not affiliated with, endorsed by, or acting on behalf of any other healthcare practice or employer of its founder or facilitators.
